Se considera el dolor pélvico crónico, aquel dolor que se presenta en la pelvis o fosas iliacas área considerada por muchas mujeres como de los ovarios, y muchas veces lo refieren como "me duelen los ovarios", con una duración mayor a 6 meses.

El medico además podrá ordenar algunos exámenes de primera mano como son el examen de orina (parcial de orina ) con o sin urocultivo y antibiograma, examen de materia fecal o coproscopico o coprologico, hemograma, creatinina, ecografía o ultrasonido transvaginal. Ya en un segundo plano en donde los exámenes iniciales no resuelven u orientan el caso podrá el medico ginecólogo solicitar un procedimiento como es la laparoscopia diagnostica y operatoria o una cistoscopia en caso de sospechar una cistitis instersticial. Con la laparoscopia se puede diagnosticar la endometriosis en cualquiera de sus tres niveles de complejidad , y de una ves tratarla , ya se a vaporizando los fosos liberando adherencias o sangrado los quistes endometriosicos o endometriomas que se encuentren durante la cirugía , además si la paciente esta interesada en fertilidad se realizara un examen llamado cromotubacion el cual se inyecta un liquido azul por la vagina para ver con el laparoscopio si sale por las trompas o por la fimbria y asi el ginecólogo laparoscopista podrá verificar que la tuba no se encuentre obstruida o este permeable , punto importante a tener en cuenta para la búsqueda posterior de embarazo. Con la cistoscopia se podrá confirmar el diagnostico de cistitis intersticial y se planteara el tratamiento. En algunos casos seleccionados se podrá solicitar una escenografía o un resonancia nuclear magnética pero no es lo corriente que se utilicen estos exámenes. En otros casos se podrá realizar pruebas terapéuticas con unos medicamentos llamados análogos de la GnRh. Según el diagnostico, el cual por lo general se aclara con los hallazgos de la laparoscopia operatoria, se enfocara el tratamiento medico, los controles y se definirá el pronostico o sea que se le explicara a la paciente como podrá ser el comportamiento de la enfermedad en el tiempo, como se realizaran los controles y el manejo de medicamentos siempre de la mano y la supervisión de su ginecólogo.
